I asked the copyright holders and all agreed to upgrade the license
from "LGPL v2.1" to "LGPL v2.1 or higher".
I did the following:
1. changed the version statement of the license in all files
2. added missing license headers to files
3. created a LICENSE file with a short decsription about what to do
Some stuff is missing:
1. many missing copyright lines (including mines) in nearly all files
2. AUTHORS is incomplete (minor issue)
